Who does CSC support?

CSC supports children and adults with intellectual and developmental disabilities and related disorders, including autism. Available services and eligibility vary by program.

How do I ask about services?

Use the secure request-services form to share your contact information and the service you are exploring. Do not submit diagnoses, medical records, Medicaid information or other health documents through the public form.

How can I apply for a job?

Visit Careers to view the application pathway and submit a résumé through the protected application workflow.

Does submitting a request confirm eligibility or placement?

No. A website request begins a conversation. Eligibility, availability, funding and referral requirements depend on the specific program and must be reviewed by CSC through the appropriate process.

Can I send medical or insurance information online?

Please do not send medical records, diagnoses, Medicaid or insurance information through a general website form. Use the form only to request a callback, then follow the secure instructions provided by CSC staff.
